Aanmelding Referral

U verwijst uw patiënt eenvoudig via het formulier. Bijlagen (röntgen/paro-status) kunt u desgewenst per e-mail sturen. You can refer your patient using the form. Attachments (x-rays/perio status) can be sent by email if desired.

Intake Intake

Na ontvangst nemen wij zo spoedig mogelijk contact op met de patiënt voor een consult en inventarisatie van de hulpvraag. After receiving the referral we contact the patient as soon as possible for consultation and intake.

Terugkoppeling & terugverwijzing Feedback & return

Aan het einde van het traject ontvangt u een terugkoppeling. De patiënt keert terug naar uw praktijk voor reguliere zorg. At the end of the pathway you receive written feedback. The patient returns to your practice for routine care.

Bijlagen kunt u per e-mail sturen. Vermeld altijd patiëntnaam en geboortedatum, zodat we uw e-mail direct aan de juiste verwijzing kunnen koppelen. You can email attachments. Always include the patient’s name and date of birth so we can match your email to the correct referral.

Wij behandelen op verwijzing en gaan zorgvuldig om met patiëntgegevens.
